The list of physicians generated is based on the search parameters you have entered (for example: city, state, zip code). If no search parameters are entered, a list of all centers who have met the listing criteria (see below) will appear in alphabetical order.

This list was created to support people who want to learn more about the Zephyr®Valve by helping connect them to physicians who currently have demonstrated qualifications, and interest in providing quality care with assessment for and treatment with the Zephyr Valve®. The information and content in the Locator are provided solely by the physicians.

The current requirements for a Center to be included in this Locator are:

Treatment Center:

• The center must have completed three Zephyr Valve cases; and

To remain on the Locator as a Treatment Center, a center must actively utilize the Zephyr Valve technology, which is defined as:

• The completion of at least 4 cases within the 12-month review period*

*The completion of a physician training course (this requirement can be waived through completion of a physician training course but cannot be waived for more than one year at a time).

Assessment Centers:

• Are able to conduct all testing procedures necessary for evaluation for treatment with the Zephyr Valve.

To remain on the Locator as an Assessment Center, a center must:

• Have referred at least two patients to a Zephyr Valve Treatment Center within a 12-month period.

The completion of the relevant criteria are assessed every 12 calendar months, with the review periods occurring January 1 – December 31. A Center may be removed from the Locator if they have not met the above requirements. A Center will be added back to the Locator once it has completed these requirements.
